Increased and custom client mapping fields

Feature Request

The client mapping fields are great to prevent completing similar fields multiple times. However, it would be nice to have more mapped fields including having custom mapped fields to be defined by the site administrator.

    I would also like to be able to add more client mapping. It would be great to be able to map anything from patient demographics into a form, specifically insurance information. This would be helpful for making forms to obtain prior auths, etc.
