Localization of forms

We have patient demographics that speak different languages. Having the option to localize a form so that various locales can be selected (let’s say from a dropdown) that would localize the form, so that a single link can be used to cover the necessary locales. At the moment, we are forced to create and manage separate forms for each locale.
